Form request tutorial //Request Form\\

First go here and sign up for the form mail.

You can use one of the codes below for your form. Make sure to change your email address to the email address you signed up with at www.javascript.nu and change the NAME="subject" to whatever subject your are wanting to show in your emial.
Also where it says redirect VALUE=yoursite.com you will need to enter a redirect page there. What that does, is after the form is submitted the user will be redirected to the page of your choice. I have mine set up to go back to my blog page. On muzikjamz I have it set up to go to a thank you page.
